RESUMO

Neonatal Jaundice is a common occurrence in neonates. High excess bilirubin would lead to hyperbilirubinemia, leading to irreversible adverse damage such as kernicterus. Therefore, it is necessary and important to monitor neonates' bilirubin levels in real-time for immediate intervention. However, current screening protocols have their inherent limitations, necessitating more convenient measurements. In this proof-of-concept study, we evaluated the feasibility of using machine learning for the screening of hyperbilirubinemia in neonates from smartphone-acquired photographs. Different machine learning models were compared and evaluated to gain a better understanding of feature selection and model performance in bilirubin determination. An in vitro study was conducted with a bilirubin-containing tissue phantom to identify potential biological and environmental confounding factors. The findings of this study present a systematic characterization of the confounding effect of various factors through separate parametric tests. These tests uncover potential techniques in image pre-processing, highlighting important biological features (light scattering property and skin thickness) and external features (ISO, lighting conditions and white balance), which together contribute to robust model approaches for accurately determining bilirubin concentrations. By obtaining an accuracy of 0.848 in classification and 0.812 in regression, these findings indicate strong potential in aiding in the design of clinical studies using patient-derived images.

OBJECTIVE: The present study was aimed at assessing the success rate and measurement value, determining the influencing factors and reference range and examining the intra-operator stability and inter-operator reproducibility of pancreatic 2-D shear wave elastography (SWE) measurement in healthy adults. METHODS: In 2022, 387 healthy adults were prospectively recruited. Logistic regression and linear regression analyses were used to explore the factors influencing the success rate and the measurement value of pancreatic 2-D SWE measurement, respectively. A two-sided 95% reference range was estimated accordingly. The intraclass correlation coefficient was calculated to evaluate the intra-operator stability and inter-operator reproducibility of the pancreatic 2-D SWE measurement. RESULTS: The pancreatic body (89.6%) bore the highest while the tail (72.8%) bore the lowest success rate of pancreatic 2-D SWE measurement. Sex and body mass index (BMI) were the independent factors influencing measurement success rate in all three parts of the pancreas. Mean measurement values (Emean) were not the same in the three parts of the pancreas of the same participant. BMI and image depth were the independent factors influencing Emean in the pancreatic body, while region of interest depth and BMI were the only independent factors influencing Emean in the pancreatic head and tail, respectively. The intra-operator stability of pancreatic 2-D SWE measurement was found to be excellent, whereas its inter-operator reproducibility was poor to good. CONCLUSION: Pancreatic 2-D SWE is a reliable technique for evaluating pancreatic stiffness in healthy adults, but its success rate and measurement value are influenced by multiple factors.

What we believe is a new scheme for producing semidiscrete self-trapped vortices ("swirling photon droplets") in photonic crystals with competing quadratic (χ(2)) and self-defocusing cubic (χ(3)) nonlinearities is proposed. The photonic crystal is designed with a striped structure, in the form of spatially periodic modulation of the χ(2) susceptibility, which is imposed by the quasi-phase-matching technique. Unlike previous realizations of semidiscrete optical modes in composite media, built as combinations of continuous and arrayed discrete waveguides, the semidiscrete vortex "droplets" are produced here in the fully continuous medium. This work reveals that the system supports two types of semidiscrete vortex droplets, viz., onsite- and intersite-centered ones, which feature, respectively, odd and even numbers of stripes, N. Stability areas for the states with different values of N are identified in the system's parameter space. Some stability areas overlap with each other, giving rise to the multistability of states with different N. The coexisting states are mutually degenerate, featuring equal values of the Hamiltonian and propagation constant. An experimental scheme to realize the droplets is outlined, suggesting new possibilities for the long-distance transmission of nontrivial vortex beams in nonlinear media.

BACKGROUND: Post-hepatectomy liver failure (PHLF) represents the major source of mortality after liver resection (LR) in hepatocellular carcinoma (HCC) patients. Child-Pugh (CP) score 5 is always considered to indicate a normal liver function but represents a heterogeneous population with a considerable number suffering from PHLF. The present study aimed to access the ability of liver stiffness (LS) measured by two-dimensional shear wave elastography (2D-SWE) to predict PHLF in HCC patients with a CP score of 5. METHODS: From August 2018 to May 2021, 146 HCC patients with a CP score of 5 who underwent LR were reviewed. The patients were randomly divided into training (n = 97) and validation (n = 49) groups. Logistic analyses were conducted for the risk factors and a linear model was built to predict the development of PHLF. The discrimination and calibration were assessed in the training and validation cohorts by the areas under the receiver operating characteristic curve (AUC). RESULTS: Analyses revealed that the minimum of LS (Emin) higher than 8.05 (p = 0.006, OR = 4.59) and future liver remnant / estimated total liver volume (FLR/eTLV) (p < 0.001, OR < 0.01) were independent predictors of PHLF in HCC patients with CP score 5, and the AUC calculated by the model based on them for differentiation of PHLF in the training and validation group was 0.78 and 0.76, respectively. CONCLUSION: LS was associated with the development of PHLF. A model combining Emin and FLR/eTLV showed proper ability in predicting PHLF in HCC patients with a CP score of 5.

Hypospadias is an abnormal ventral development of the penis caused by incomplete virilization of the male genital tubercle. This study investigated the phenotypic modulation of vascular smooth muscle cells (VSMCs) in the corpus spongiosum surrounding the urethral plate in hypospadias. The urethral corpus spongiosum tissue was collected for HE, Masson and α-SMA immunohistochemical staining. Spongiosum VSMCs were cultured and identified by α-SMA fluorescence. qRT-PCR and Western blotting and fluorescence were performed. The results showed that the vascular lumen of the corpus spongiosum around the urethral plate was larger and that the vascular smooth muscle layer was thicker in hypospadias. The expression of the contractile markers α-SMA and Calponin 1 in VSMCs was decreased, the expression of the synthetic marker OPN was increased, and the transcription of the phenotypic switching factors SRF and MYOCD was decreased. The expression of Ki67, PCNA and BAX was increased, and the expression of Bcl-2 was decreased. The phenotype of corpus spongiosum VSMCs in hypospadias changed from the contractional type to the synthetic type. This phenotypic modulation was associated with increased proliferation and apoptosis rates. SRF and MYOCD may be the main factors mediating the phenotypic modulation of urethral corpus spongiosum VSMCs.

Atmospheric pressure chemical vapor deposition (CVD) is presently a promising approach for preparing two-dimensional (2D) MoS2 crystals at high temperatures on SiO2/Si substrates. In this work, we propose an improved CVD method without hydrogen, which can increase formula flexibility by controlling the heating temperature of MoO3 powder and sulfur powder. The results show that the size and coverage of MoS2 domains vary largely, from discrete triangles to continuous film, on substrate. We find that the formation of MoS2 domains is dependent on the nucleation density of MoS2. Laminar flow theory is employed to elucidate the cause of the different shapes of MoS2 domains. The distribution of carrier gas speeds at the substrate surface leads to a change of nucleation density and a variation of domain morphology. Thus, nucleation density and domain morphology can be actively controlled by adjusting the carrier gas flow rate in the experimental system. These results are of significance for understanding the growth regulation of 2D MoS2 crystals.

Propagation of Airy-Gaussian vortex (AiGV) beams through the gradient-index medium is investigated analytically and numerically with the transfer matrix method. Deriving the analytic expression of the AiGV beams based on the Huygens diffraction integral formula, we obtain the propagate path, intensity and phase distributions, and the Poynting vector of the first- and second-order AiGV beams, which propagate through the paraxial ABCD system. The ballistic trajectory is no longer conventional parabolic but trigonometric shapes in the gradient-index medium. Especially, the AiGV beams represent the singular behavior at the propagation path and the light intensity distribution. The phase distribution and the Poynting vector exhibit in reverse when the AiGV beams through the singularity. As the order increases, the main lobe of the AiGV beams is gradually overlapped by the vortex core. Further, the sidelobe weakens when the AiGV beams propagate nearly to the singularity. Additionally, the figure of the Poynting vector of the AiGV beams proves the direction of energy flow corresponding to the intensity distribution. The vortex of the second-order AiGV beams is larger, and the propagation velocity is faster than that of the first order.

The propagation of right-hand circularly polarized Airy-Gaussian beams (RHCPAiGBs) through slabs of right-handed materials (RHMs) and left-handed materials (LHMs) is investigated analytically and numerically with the transfer matrix method. An approximate analytical expression for the RHCPAiGBs passing through a paraxial ABCD optical system is derived on the basis of the Huygens diffraction integral formula. The intensity and the phase distributions of the RHCPAiGBs through RHMs and LHMs are demonstrated. The influence of the parameter χ0 on the propagation of RHCPAiGBs through RHM and LHM slabs is investigated. The RHCPAiGBs possess transverse-momentum currents, which shows that the physics underlying this intriguing accelerating effect is that of the combined contributions of the transverse spin and transverse orbital currents. Additionally, we go a step further to explore the radiation force including the gradient force and scattering force of the RHCPAiGBs.

RESUMO

A general, organocatalytic inverse-electron-demand [3+2] cycloaddition reaction between a range of carbonyl compounds and diazoacetates has been developed. This reaction is catalyzed by secondary amines as a "green promoter" to generate substituted pyrazoles with high levels of regioselectivity. It is noteworthy that this [3+2] cycloaddition reaction proceeds efficiently at room temperature with a simple and inexpensive catalyst. Considering the large variety and ready availability of the starting materials (e.g. ketones, ß-ketoesters, ß-diketones, and aldehydes), as well as the operational simplicity of this process, a convenient, practical, and highly modular pyrazole synthesis has been developed. We believe that this work will arouse more research interest in the organocatalytic synthesis of other biologically active heterocycles. Such studies are currently underway in our laboratory.
